We offer flexible, person-centred outreach support for individuals with Learning Disabilities and/or Autism Spectrum Disorder. Our outreach services are designed to promote your independence at home and help you actively participate in your community.

How It Works

Hours and focus, set by you

Outreach support is flexible by design. Together we agree how many hours suit you, which goals to focus on first, and how we'll know it's working — then we review and adjust as your confidence grows.

  • Sessions scheduled around your routine, not a fixed rota.
  • Consistent support workers who get to know you properly.
  • Regular reviews with you and your support network.
